City and Hackney PCT is now part
of a cluster of three primary care trusts called NHS East London

We want to help everyone in Hackney and the City to improve
their health, and this means offering the right care at the right
place and the right time. Sometimes the right place may be in a
'one stop shop', your child's school, or even your home.
Below is a list of the NHS City and Hackney's community health
services. Most of the time your GP will refer you to one of these
services when you need to see a particular professional like a
therapist or a community nurse. However, some of these services do
allow you to get help directly without seeing your GP - these
services are identified with an asterix (*).
