Adolescent health service (CHYPS Plus)

CHYPS Plus

The CHYPS Plus service aims to provide holistic services for 11 to 19 year olds through direct work, group work, consultation and training from a multi-disciplinary skill mixed team. The team works alongside the core school health service to deliver targeted services in school settings, as well as targeted and specialist services from other settings, including the service hub and the House.

 

Objectives

  • To deliver a holistic service using the wider team of agency partners supporting the delivery in key areas e.g. disability, substance misuse, moderate to severe mental health, support for young parents
  • Deliver services from a range of health and non-health settings, including the House (hub), schools and colleges, the youth service and YOT, Huddleston disability centre and other locations
  • Target young people who are most vulnerable to poorer health outcomes
  • Deliver against key public health targets including sexual health, physical health, obesity and health eating, emotional health, and long term conditions
  • Service to undertake lead role in promoting and ensuring youth participation and involvement in line with Hear by Rights framework for young people
  • Deliver against standards set for You’re Welcome quality criteria, for making clinical and non-clinical youth settings young people-friendly

 

Referrals

Referrals can be made by parents or guardians, or by any professional working with young people. Patients can self refer to this service.

All referrals made to the service will receive a response within 24 hours.

 

Opening hours

The House (Lower Clapton) is open Monday to Friday from 9am–5pm. Drop-ins and outreach work takes place in a number of locations across the borough. If you are interested in receiving support for young people in any setting, please contact us on 0208 919 5040.
