Adult community nursing

The Adult Community Nursing Service provides
community-based nursing care, case management and advice to
patients who are aged 18 years and over and primarily
housebound. Patients must have needs that require assessment and/or
ongoing intervention by a trained nurse. Care may also be provided
to ambulatory clients who require nursing care that is unable to be
provided by practice nursing, and post-operative procedures like
removal of sutures.
The service is made up of several components parts:
District Nursing Service
has eight teams, two in each of the four quadrants
of the City and Hackney (NE, NW, SW, and SE
localities).
The Discharge Planning and Long-term
(Continuing) Care Service, which supports discharge for
people with more long-term care needs.
Community Matrons, who
provide a case management service for people with long term
conditions, and work very closely with the team-based
service.
Clinical Nurse Specialists in
the areas of Tissue Viability, Continence, Heart Failure,
Palliative Care, Respiratory Care, Parkinsons’ Disease, and
Multiple Sclerosis.
First Response Duty Team,
which is a jointly-funded team with social services; nurses here
work alongside the community nursing teams.
The aims of the service are to improve or
maintain health, and promote independence by providing high quality
nursing care to patients in the community setting. This is achieved
by working in partnership with patients, their carers and other
health and social care professionals to ensure that, wherever
possible, care is maintained at home.
The adult community nursing service provides
the following services:
- First contact/acute assessment, diagnosis, care, treatment and
referral
- Co-ordination and management of care provided for patients with
complex health needs
- Continuing care, rehabilitation, long term condition
management, and delivery of national service frameworks
- Assessment for nursing equipment
- Continence assessment and support
- Wound assessment and care
- Palliative care for the terminally ill, including bereavement
support for carers
- Public health/health promotion and protection programmes that
improve health and reduce health inequalities.

Patients can self refer or be referred by a
GP, hospital, or health or social care professional. Referrals
are made by FAX
