The Adult Community Rehabilitation Team (ACRT) is a forward-thinking service that provides interdisciplinary rehabilitation to adults over the age of 16. Our aim is to enable people to be as independent as possible.

The team consists of physiotherapists, occupational therapists, a speech and language therapist, a neuropsychologist, a disability counsellor, rehabilitation assistants and administrative support. 
ACRT will see clients in the environment most convenient to them, whether it is their own home, nursing/residential home, place of work/study or on-site at St Leonard's.

Examples of our services

  • Giving advice to help people cope with specific problems or crises
  • Helping people come to terms with the impact of their disability
  • Helping a person to make informed choices on the management of their disability
  • Helping increase someone's confidence and independence in looking after themselves
  • Supporting carers and family members
  • Helping people to access community services such as leisure centres and voluntary organisations
  • Preventing admission to hospital
  • Helping people to return home following a hospital admission
  • Various groups are run to maximise clients' rehabilitation experience

Referrals

Patients can be referred to the service by any health or social care professional. 

People with progressive conditions such as Multiple Sclerosis or Parkinson's disease, or those clients who are already known to the team, can self-refer. If the client is not previously known to the team, then a GP or consultant referral may be required.
