Children and Families Nursing

Children and families nursing services provide primary prevention and secondary treatment-based services to respond to the needs of all children aged 0-19 years, and for children looked after by the London Borough of Hackney or Corporation of London. This includes children with disabilities, life-limiting conditions, and complex health needs.

Services

  • Work in partnership with general practice, and partner agencies to develop integrated childrens services
  • Ensure that all children and young people aged 0-19 years are offered the child health promotion and immunisation programme
  • Enable, support and safeguard the interests of vulnerable children who are looked after, have complex health needs, are homeless or have a chronic condition like sickle cell and thalassaemia disease; ensure they can access primary and secondary care services appropriately
  • Work in partnership with parents and carers to offer care packages supported by an individualised care plan that meets the assessed needs of a child, young person or their parent/carer
