Children's community nursing

The purpose of the community children's nursing team is to
enable children with chronic or complex health needs to receive
nursing care within their own home.
Services
- Support for families and children with long-standing nursing
needs arising from chronic or complex medical conditions
- Performing agreed nursing tasks for children with acute nursing
needs in agreed circumstances with tertiary centres, for
example a course of IV therapy at home
- Follow-up of premature babies, including support with home
oxygen and enteral feeding
- Support for families and carers caring for terminally ill
children
- Establish with social services a care package, which will
provide for the growth, development, education, psychological and
emotional wellbeing of the child
- Confirm, in partnership with parents, their learning needs to
enable them to take responsibility for their child's needs
Referrals
Referrals can be made to the service by local hospitals,
paediatricians, other children’s community nursing teams and other
healthcare professionals.
Opening hours
Monday to Friday from 9.00am - 5.00pm.
A 7-days-a-week service is provided for children receiving
complex care packages.
