Physiotherapy service (Locomotor)

Locomotor is NHS City and Hackney's physiotherapy service, and
it provides specialist assessment and treatment for adults
with musculoskeletal conditions (muscles, joints, nerves, discs)
affecting the spine, arms and legs. We can help a wide variety of
conditions from acute problems such as sports injuries and whiplash
to longer term conditions such as back pain or arthritis.
We operate a single point of entry so patients are seen by the
right person, in the right place at the right time, without the
need for referral elsewhere. Patients are seen in the physiotherapy
service by physiotherapists, extended scope practitioners,
specialist GP and the Pain Service or referred to hospital
consultants (secondary care). Clinicians can refer patients to
other members of the team to ensure seamless care.
Physiotherapy aims to restore movement and independence, giving
patients the power to understand and manage their own bodies. We
follow evidence-based practice, using the 'bio psychosocial model
of care' which holistically considers the physical, medical,
psychological and social factors. In other words, we care for the
whole person.
Physiotherapists work in health centres such as St Leonard's and
run assessment clinics in GP practices. There is a choice of
appointment, location, treatment and same sex therapist.
After individual assessment, physiotherapy offers a wide range
of treatments such as manual therapy (manipulation and other
hands-on techniques), acupuncture, myofascial release, and/or
exercise therapies individually in our gymnasium or groups.
Physiotherapy-led classes run at the local leisure centres
include: back pain classes, Turkish back group, pilates, functional
restoration classes and osteoarthritis knee classes. Also available
is hydrotherapy and combined physiotherapy / podiatry clinic.
Extended scope physiotherapists have expert treatment and
diagnostic skills for complex problems and clinically support other
physiotherapists. They are trained to order investigations (MRI,
x-rays, blood tests) and incorporate ultrasound imaging and joint
injections into their practice.
The Pain Service
The pain team cares for patients with longstanding and severe
musculoskeletal pain. It is based within the St Leonard's
physiotherapy department, and provides assessment and management of
complex pain using a bio-psychosocial model of care.
Our interdisciplinary team includes pain consultants, GP,
physiotherapists (Biomedical Team), pain nurse and the psychosocial
team of occupational therapists and psychologists.
Patients receive individual assessment and treatment by a
physiotherapist, and follow-up investigation of their condition
where necessary.
Our team can do further investigation of complex pain and can
also refer patients to other specialists, such as
neurosurgeons.
The psychosocial team aims to enhance the patient's ability to
live a life of quality, despite persistent pain. This is achieved
through development of helpful strategies that counter the effects
of persistent pain in the practical, social and emotional areas of
life. The psychosocial team also run a pain management group
programme with the input of other members of the team. Patients who
have completed the pain management programme can also attend a
regular support group.
Patients with a long-standing condition may periodically have
new challenges. We welcome our patients to continue to use the pain
service as a resource to support them in their self management.

Referrals
Referrals are accepted from GPs, consultants, physiotherapists
and other health professionals.
