Oldhill practice docs
NHS City & Hackney invites Expressions of
Interest from all suitably qualified and experienced healthcare
providers (including general practitioners, social enterprise /
third sector organisations and other providers) to provide a range
of primary care, unplanned care and enhanced / additional services
as part of the NHS ‘Equitable Access to Primary Medical Services’
initiative for two separate schemes..
Potential providers will be expected to enter into an APMS
contract for a term of 7 years with a five year break clause.
The services currently sought, are as follows:
South East Hackney scheme
A GP-led Health Centre to be located at a new Primary Care
Resource Centre in the South East area of Hackney (E9) when
building is completed in the autumn of 2010. The practice is
expected to grow to approximately 4,000 patients.
North East Hackney scheme
A GP-Led Health Centre to be based on the site of the existing
Oldhill practice in North East Hackney (N16) currently under
temporary provision arrangements and with a current list size of
approximately 4,000 patients with a potential list growth to
approximately 6,000 patients. The GP-led health is designated to
move into the North East Resource Centre when a site is identified
and construction completed.
Both GP-led Health Centres are expected to provide:
High quality primary care services responsive to the diverse
needs of the local multi-ethnic community.
- Extended GP service opening hours from 8am to 8pm, 7 days a
week.
- Walk-in and appointment facilities for both registered and
unregistered patients
- Participate fully in health promotion and developing the
healthy lifestyle of patients while tackling the underlying causes
of ill health.
- Participate fully in the development of networked polyclinic
models based on NHS City & Hackney’s Primary Care Resource
Centre strategy and consistent with Healthcare for London
models.
- Develop links to local schools, minority, ethnic and
underserved groups.
- Create operational links with the co-located health, social,
community and voluntary
- service teams as part of an
integrated service.
It is expected that both contracts will be signed in December
2009. It is also expected that the North East Hackney scheme
commences in January 2010. The mobilisation timetable for the South
East Hackney scheme will be subject to further discussions but the
service is expected to open in the autumn of 2010.
The proposed schemes are subject to on-going public and
stakeholder engagement.
